Massage Facility Application

To obtain a Massage Therapy License in the City of Grandview, the Massage Therapist must submit a copy of their State Massage Therapy License, which will be verified by the City Clerk. An application for a Grandview Massage Therapy License must be completed, which can be provided by the City Clerk. A fee of $30.00 is required at the time the application is submitted. As part of the application, the Massage Therapist must provide the facility in which their massage will be performed. That facility’s owner must also obtain a State Massage Therapy Business License (Grandview refers to this license as a Massage Therapy Facility License).

To obtain a Massage Therapy Facility License for the City of Grandview, the owner of the facility must submit a copy of their State Massage Therapy Business License, which will be verified by the City Clerk. An application for the City’s Massage Therapy Facility License must be completed, which can be provided by the City Clerk by calling (816) 316-4811. A fee of $100.00 is required at the time the application is submitted. The City Clerk will order a building inspection by the City’s Building Official. Once the facility has passed inspection by the City, a license may be issued by the City Clerk.
